Required for riders 17yr or younger attending a practice or race at ACP. You may complete this form on-line before you print. Or, you can print and then complete the form. The form needs to be notarized if the parent or legal guardian will not be present.

We have an Annual Minor Release available. The release is valid for a calendar year only. So, a new Annual Minor Release will need to be complete at the beginning of new year. The form must be printed on one page (Front/Back 1 page) and it must be notarized with the rider’s signature and both parents/guardians to be valid.

Temporary Guardian / Medical Consent Form (Required for AMA Events): Required for riders 17yr or younger attending a practice or race at ACP without a Parent or Guardian. We all would like to assume that nothing will happen, but you just never know. Having this information readily available will help expedite medical attention.

CMC Copper State Entry Forms: This form must be printed on ONE piece of paper. There is a front and back. The form will not be accepted stapled together or as two pages.
