Medical Consent Form: This form is recommended if a rider that is 17yr or younger will be attending a practice or event without their parents or legal guardian.  This form will help expedite emergency care for your child. This form will be used in conjunction with the Parental Consent Form.

ARIZONA CYCLE PARK DAILY MINOR WAIVER:

ARIZONA CYCLE PARK 2024 ANNUAL MINOR WAIVER:

Both parents signatures are required (unless you have sole custody which must be notated) and notarized.  These forms can be picked up at the track as well.

Required for riders 17yr and younger

Parental Consent Form: This form will be needed if a rider that is 17yr or younger will be attending a practice or event without their parents or legal guardian.  This form must be printed with both page 1 & page 2  on one piece of paper (front & back).  This form cannot be accepted if pages are stapled.  This form needs both front & back and notarized.

AMA  EVENT MINOR WAIVER:

Both parents signatures are required and must be notarized if parents are not going to be present at event.
